Boonsboro nursing home sold for $15M

BOONSBORO - The former Reeders Memorial Home has new owners, a new name and a new rating for quality.

The nursing home at 141 S. Main St. in Boonsboro has been renamed Sterling Care at South Mountain. It now is owned and operated by Sterling Senior Care. Nathan Jakobovits of Baltimore and Jeff Kagan of Silver Spring, Md., own Sterling Senior Care.

"This is a new venture for myself and Jeff," Jakobovits said Friday. "We've been in this industry for a number of years."

South Mountain SNF Realty LLC, a real-estate arm of Sterling Care, purchased the facility from FC-THC Leasing LLC for $15,553,615, according to Washington County property records.

The transfer is dated March 9 on county records. But Jakobovits and Kagan said the deal, which includes the property, equipment and the business, went into effect Jan. 1.

Jakobovits and Kagan both are licensed nursing-home administrators with experience working in long-term-care facilities.

Sterling Senior Care also recently bought properties in Frostburg, Md., and Salisbury, Md., now known as Sterling Care at Frostburg Village and Sterling Care At Harbor Pointe, respectively.

"Our rule is we have to be able to drive there within a day," Jakobovits said. "We're both intimately involved in our facilities. We're in our facilities on a weekly basis."

Rankings of nursing homes are published on the Medicare.gov website. Jakobovits said Reeders' overall rating had fallen to one star, the lowest rank, a few years ago.

"We saw this as an opportunity to come in and enhance the services," he said.

In recent months, staff members have instituted several changes, he said, and more improvements have been made since Jan. 1.

Now, Sterling Care at South Mountain has an overall rating of three stars. The new owners said staff members are responsible for much of that improvement.

"That's something we're very proud of," Jakobovits said.

Jakobovits and Kagan said their goal is to reach the highest rating, which is five stars.

"We feel that the way to be successful is to provide the quality care," Jakobovits said.

The facility has 148 beds for skilled nursing care and rehabilitation services - what Jakobovits called the "full suite of skilled services." It also has a 16-unit assisted-living center. Sterling Senior Care has plans for further upgrades, including investments in the gym and in an electronic medical-records system.

More than 200 people work at Sterling Care South Mountain. The new owners said they have introduced some operational changes, including regular town-hall-style meetings to get input and ideas from staff members.

"Our staff (to patient) ratios are above the state average," Jakobovits said.

The parcel includes 34.7 acres, according to Washington County property records.
